MEMORIAL INVITATIONAL GOLF TOURNAMENT

CHESTERLAND, OHIO

Entry Fee $150 per person ($115 tax-deductible). The entry fee includes 18 holes of golf & cart, unlimited Range Balls prior to play, a FMGC personalized Bag Tag, Bag Drop Service, Hot Dog or Hamburger, Chips & Fountain Lunch, a “Goodie Bag” with a Golf Hat, Golf Balls, Bottled water & Snacks; Golf Tees, Divot Tool and Cigars at Registration. Keg Beer, Soda and Fruit available during golf on the course. Hors d’oeuvres, Cash Bar and a Buffet Dinner after golf.

Entries will be on a first come basis, so please return your completed entry immediately. The field will be limited to the first 144 golfers. Enter early! Make your own team or we will pair individual players. For your convenience we have enclosed a return envelope for your entry.

The golfing event will be an 18-hole foursome modified scramble. Mulligans are available at Registration. Prizes will be awarded for both low net and low gross through two places. Prizes will also be awarded for skill shots for the Ladies and Men: these include Hole-in-one, closest to the pin, long drive and putting.

All proceeds from this event go directly into the Yoder Brothers Foundation Scholarship Fund. This Foundation, established in memory and celebration of Joshua and Tyler, is dedicated to providing financial support for students pursuing post- secondary education.
